There was once an Asura named Hayagreeva who had obtained a boon from Devi (Shakti) that he may be killed only by a person having the head of the horse. When his reign of oppression became unbearable, the Devi contrived that Vishnu's head be lost. Brahma then replaced the head of Vishnu with one of a horse.
This horse faced creature then fought with the Rakhshasha Hayagreeva and killed him. Since he had the face of a horse, this incarnation of Vishnu is also referred to as Hayagreeva.
The story can be found in the Devi Bhagata here.
